Confirm the time, location and transport, ask your parent what they want to discuss, and take relevant letters and an up-to-date medication list. Let a clinician answer clinical questions.
Before you go
Write down your parent's questions in their own words. Check whether they want you in the room and how the practice handles consent and information sharing.
After the appointment
Record what was actually agreed, any follow-up date and who will do it. If instructions are unclear, ask the clinical team to clarify rather than interpreting them yourself.
A practical checklist
- Confirm date, location and transport
- List questions and bring relevant letters
- Bring the current prescribed medication information
- Write down confirmed follow-up tasks and contacts
Official guidance and further reading
Check these organisations for the latest rules and guidance for your location.
Frequently asked questions
- Can I speak for my parent at the appointment?
- Ask your parent what involvement they want and check the practice's consent arrangements. The clinician can explain how information sharing works in their situation.
