NHS continuing healthcare (CHC) is a package of care arranged and funded by the NHS in England for adults whose main need is a health need. Eligibility is decided through an NHS assessment, not by a diagnosis alone.

How it starts

Usually a health or social care professional completes an initial checklist. If that suggests someone might be eligible, a fuller assessment follows. You can ask the GP, social worker or hospital team about it.

What families can do

Keep a clear, factual record of the care and support the person receives day to day, and the names of the professionals involved. Ask for written copies of decisions.

If the answer is no

Ask how the decision was made and what the next steps are, including whether NHS-funded nursing care or council support may apply.
