What information should a care handover include?
Part of Organising care

A useful handover records what happened, what is confirmed next, what remains unresolved and who owns each action. Keep it factual, dated and limited to information the recipient is allowed to see.
Separate facts from follow-ups
Write 'appointment booked for Tuesday' only if confirmed. If you are waiting for a call, say so and give the person taking over a next step.
Keep it appropriately private
Share details on a need-to-know basis. A family handover is not a clinical record or an instruction to change treatment.
A practical checklist
- Date, time and person handing over
- Completed practical tasks and source of information
- Confirmed appointments and who will attend
- Open questions and named follow-up owner
- Relevant contact and document location, where permitted

Frequently asked questions
- Should a handover contain medical advice?
- No. Record what a professional actually said or where the official instruction is stored; ask that professional to clarify anything uncertain.
